Information notice on the processing of personal data

The personal data you provide to us is processed in compliance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data (hereinafter the "GDPR").
The data controller is the Ministry of Agriculture, Food and Viticulture - Directorate for Consumer protection (hereinafter the "DPC") located at 271, route d'Arlon, L-1650, Luxembourg and the Centre des technologies de l'information de l'État (hereinafter the "CTIE") located at 560, rue de Neudorf, L-2220, Luxembourg is its main subcontractor.
Your personal data, collected in the form (hereinafter "the form") on a voluntary basis, will be processed by the DPC in order to send you a "Non au colportage" sticker by post.
When you fill in the form and click on the "send" button, the form is sent directly to the site of the CTIE's subcontractor, surveygizmo.eu from Alchemer LLC, based in Your personal data, collected on the form (hereinafter "the form") on a voluntary basis, will be processed by the DPC in order to send you a "Non au colportage" sticker by post. When you fill in the form and click on the "send" button, the form is sent directly to the site of the CTIE's subcontractor, surveygizmo.eu from Alchemer LLC, based in the United States, 4888 Pearl East Circle, Suite 100W, Boulder, CO80301 (https://www.alchemer.eu/). Personal data will be stored for the duration of the "Non au colportage" sticker mailing, plus 2 months to enable DPC to mail the stickers to the address you have provided, and for subsequent deletion of your data.  
In accordance with the GDPR, you benefit from (i) a right of access, (ii) a right of opposition except in cases where the processing of data is compulsory, (iii) a right of rectification, (iv) a right to the deletion of your data and (v) a right to the limitation of processing.
